Surgical Safety
All hospitals in Ontario providing surgical care to their clients are participating in the Surgical Safety Checklist Program. This program ensures that the right information is available to the right members of the health care team at the right time.
The checklist consists of three phases: the briefing, time out and debriefing. All three phases must be completed on each client entering the Operating Room. The completion of the three phases will be the indicator for our compliance rates. These rates will be posted monthly on the web site as part of our patient safety program.
October 2011 rates
16 completed checklists = 100% compliance in the month of October
16 surgical patients (Surgery date October 4 & 5, 2011)
April 2011 rates
15 completed checklists = 100% compliance in the month of April
15 surgical patients (Surgery date April 11 & 12, 2011)
November 2010 rates
8 completed checklists = 100% compliance in the month of November
8 surgical patients (Surgery date November 1& 2, 2010)
July 2010 rates
15 completed checklists = 100% compliance in the month of July
15 surgical patients (Surgery date April 2010)
